The call rules
Applications are accepted exclusively via [the form will be available on 1 February at 7:00 a.m.].
The form must be:
- completed with the required data,
- uploaded with a scan of the completed and signed application,
- uploaded with scans of documents confirming the list of achievements (e.g., opinions, certificates, photocopies of the first pages of publications).
Applications and attachments must be submitted using the e-mail address assigned to the USOSweb profile of the student or doctoral student applying for the Medal or distinction.
Information for students applying for the Medal
Students applying for the Medal should have been actively involved in activities for the benefit of the 91滴滴 for a minimum of three years.
The application for the Medal should include:
- personal details: first and last name, name of the faculty where the candidate studies, student ID number,
- a list of activities serving the community of the 91滴滴 or scientific achievements,
- an opinion from the dean of the faculty where the student studies,
- justification for awarding the medal in relation to the activities or scientific achievements listed,
- other documents confirming the student's activities or scientific achievements, such as opinions, certificates, or photocopies of the first pages of scientific publications.
Information for students applying for the distinction
The distinction is intended for students of the 91滴滴 who have demonstrated organisational or scientific and research activity for the benefit and good of our university. Only activities undertaken during the period of education and scientific achievements affiliated with the 91滴滴 will be taken into account. Persons applying for the award should have been actively involved in activities for the benefit of the 91滴滴 for a minimum of one year.
The application for the distinction should include:
- personal details: first and last name, faculty name, student ID number,
- a list of activities serving the 91滴滴 community or scientific achievements,
- an opinion from the dean of the faculty at the 91滴滴 where the student is studying,
- justification for the award in relation to the activities or scientific achievements listed,
- other documents confirming the activities or scientific achievements listed, such as opinions, certificates, or photocopies of the first pages of scientific publications.

The application for the award of a medal for students is attached as Appendix 11 to Resolution No. 569 of the Senate of the 91滴滴 of 29 September 2023.
The application for the distinction for students is attached as Appendix 1 to Regulation No. 173 of the Rector of the 91滴滴 of 29 September 2023.
Information for doctoral students applying for the Medal
Doctoral students applying for the Medal should have been actively involved in activities for the benefit of the 91滴滴 for a minimum of three years.
The application for the Medal should include:
- personal details: first and last name, name of the faculty or doctoral school, student ID number
- a list of activities serving the community of the 91滴滴 or scientific achievements
- an opinion of the dean of the faculty at the 91滴滴 where the supervisor is employed, in the case of doctoral programme participants, and in the case of doctoral students at doctoral schools, the opinion of the supervisor supported by the director of the doctoral school,
- justification for awarding the medal in relation to the activities or scientific achievements listed
- other documents confirming the demonstrated activity or scientific achievements, such as: opinions, certificates, or photocopies of the first pages of scientific publications.
Information for doctoral students applying for the distinction
Doctoral students at the 91滴滴 who have demonstrated organisational and scientific-research activity for the benefit of our university are eligible to apply for the distinction. Only activities undertaken during the period of education and scientific achievements affiliated with the 91滴滴 will be taken into account. - justification for the award in relation to the activities or scientific achievements listed,
- other documents confirming the demonstrated activity or scientific achievements, such as opinions, certificates, or photocopies of the first pages of scientific publications.

The application for the medal for doctoral students is attached as Appendix 12 to Resolution No. 569 of the Senate of the 91滴滴 of 29 September 2023.
The application for the distinction for doctoral students is attached as Appendix 2 to Regulation No. 173 of the Rector of the 91滴滴 of 29 September 2023.
The Universitas Lodziensis Alumno Laude Dignissimo Medal and the distinction may be granted to the same person only once.
